Chapter 16, Section 4 (פֶּרֶק ט״ז אוֹת ד׳)

4. And when a person would seek to descend to the Merkavah (ד. וְכֵיוָן שֶׁהָיָה אָדָם מְבַקֵּשׁ לֵירֵד לַמֶּרְכָּבָה), he would summon Suriy"ah, the Prince of the Presence (הָיָה קוֹרֵא אוֹתוֹ לְסוּרִי״ה שַׂר הַפָּנִים), and adjure him one hundred and twelve times (וּמַשְׁבִּיעוֹ מֵאָה וּשְׁתֵּים עֶשְׂרֵה פְּעָמִים), by Totrosiya"y Hashem (בְּטוֹטְרוֹסִיאָ״י ה׳), who was called Totrosiya"y (שֶׁהָיָה נִקְרָא טוֹטְרוֹסִיאָ״י), Tzurat"k (צוּרַט״ק), Totrakhiya"l (טוֹטְרַכִּיאָ״ל), Topgar (טוֹפְג״ר), Ashruviliya"y (אַשְׁרוּוִילִיאָ״י), Zevudiya"l (זְבוּדִיָּא״ל), Zohareriya"l (זֹהַרְרִיָּא״ל), Tanda"l (טַנְדַּ״ל), Shokad (שׁוֹק״ד), Huzi"ah (הוּזִי״אָ), Dehiburiyan (דְּהִיבּוּרִי״ן), and Adirirun (וַאֲדִירִירוּ״ן), Hashem, God of Yisrael (ה׳ אֱלֹקֵי יִשְׂרָאֵל).
